I found this link and it does offer a wealth of information. I plan to design my own forms (I use Microsoft Office Professional), but this site gave examples. Luckily, I have an awesome and experienced teacher/mentor to help with these kinds of things, and my first fundraiser will just be small (approx. 10 cheerleaders raising $$$ for national competition costs.) But no matter the size, I still want to have all my paperwork be concise, legal, and accurate.
