Or maybe you know of an address where any of these can be found. or possibly even you could just say what you have listed on your forms.for instance what info is needed for a wholesale account--other company's name, contact name, address, phone number, etc...... I guess in part what I was hoping for was maybe a layout of sorts. The fundraising forms will be easier for me to make since I've been involved in fundraising with school and organizations.But wholesale and consignment that's a whole other fish for me.
