I know I ordered something from Lotioncrafters once and she ran my card twice and it said something similar to yours. She thought maybe she had the wrong expire date or something so she called me. I told her all the info was correct. She ran the card a 3rd time and it went through. She said she didn't understand it and that maybe her gateway was doing crazy things. She also told me she doesn't run a card more than twice because every time she enters it, it charges her. When I told her all the info and numbers were correct, she said she'd run it one more time and if it didn't work I could use another card or call my bank. That's the only time I've had that happen to me. So, you might give your bank a call and see if they can tell you what happened based on the ref. # she gave you. Good Luck!

In Texas, it's required to have a "hard" copy of the Tax ID or Resale Certificate on file to be tax exempt. If they are legitimate, they shouldn't mind providing that to you.

Sorry, I haven't seen that size. You might try asking someone at Office Max, Office Depot or Staples. They suppliers they can order from if they don't carry it in the store. Kinko's might be able to help too. Good luck!
